Cancellation Policy

FullScope Mobile Diagnostics LLC – Booking Policy FullScope Mobile Diagnostics LLC provides mobile drug testing, instant oral fluid testing, phlebotomy services, and wellness specimen collection across Odenville, St. Clair County, and surrounding Alabama areas. To ensure smooth scheduling and exceptional service, the following booking policies apply to all appointments: 1. Scheduling Appointments may be booked online, by phone, or by email. Your appointment is considered confirmed once you receive a confirmation text or email. Same-day and urgent bookings are accepted based on availability. 2. Mobile Service Area We travel throughout St. Clair County, Jefferson County, and surrounding areas. Locations outside our primary service zone may include a travel fee depending on mileage and time. 3. Payments Payment is due at the time of service unless a corporate client agreement is in place. Some services may require a non-refundable deposit to secure your appointment. Invoices are available for employers and repeat clients. 4. Cancellation & Rescheduling Cancellations or appointment changes must be made at least 12 hours prior to the scheduled time. • Late cancellations may incur a $25 fee. • No-shows or client unavailability upon arrival will result in full service fees. 5. Late Policy There is a 10-minute grace period. If the client is not available beyond this window, the appointment may be marked as a no-show and fees may apply. 6. Identification Requirements A valid ID (driver’s license, state ID, passport, or employer badge if pre-authorized) must be provided before any specimen collection. Services will not be performed without proper identification. 7. Workplace & Third-Party Testing Employers must provide proper documentation (chain-of-custody forms, authorization forms, or agency referral paperwork). A private, safe testing area must be available at the location. 8. Compliance & Safety FullScope Mobile Diagnostics LLC follows strict chain-of-custody protocols, industry standards, and HIPAA privacy rules. Services may be refused for unsafe environments, harassment, or signs of intoxication. 9. Results Instant test results are provided on-site. Lab-based testing results typically return within 24–72 hours depending on the test type. Results are released only to the client or authorized employer/agency. 10.
